Long-term care planning can be overwhelming for individuals and families navigating aging-related challenges. The Long-Term Care Planning Assistant plays a vital role in supporting older adults and their loved ones by helping coordinate care-focused planning, gathering critical information, and ensuring each client experience is handled with compassion, clarity, and professionalism.

Essential Duties and Responsibilities
- Support Long-Term Care Planners and attorneys with all aspects of long-term care and Medicaid-related planning.
- Compile, organize, and maintain detailed client information.
- Assist in preparing and tracking long-term care and Medicaid documentation.
- Attend client meetings with planners and attorneys.
- Communicate with long-term care facilities and caseworkers.
- Serve as a witness and/or notary when needed.
- Maintain electronic and hard-copy files.
- Prepare and proofread correspondence.
- Answer and route phone calls appropriately.
- Perform general office and administrative tasks.
- Maintain strict confidentiality.
- Perform other duties as assigned.
Qualifications
- 2-3 years of experience in caregiving, social work, long-term care, or related fields.
- Bachelor's degree preferred or equivalent experience.
- Strong organizational and communication skills.
- High level of empathy and professionalism.
- Ability to work independently and collaboratively.
- Proficiency with Microsoft Office.
Compensation is based on experience.
