Geriatric Case Manager
Naples, Florida
We’re hiring a Geriatric Case Manager in Naples, Florida who wants meaningful work, a predictable weekday schedule, and strong benefits.
You’ll step into a community-based role focused on supporting older adults and their families through both short- and long-term case management. You’ll develop individualized care plans, conduct home visits, and maintain ongoing communication with your caseload to help clients remain independent. You’ll have autonomy in managing your schedule while supported by a dedicated team.
You’ll practice in Naples, Florida, a coastal community known for its beautiful beaches, active lifestyle, and strong sense of community.

Compensation & Benefits
• Salary of $55,000
