Hospice Clinical Manager (RN) Shift Available: Full Time Service Area for Coverage: Greater Houston Responsibilities:

• Oversee the multidisciplinary team providing hospice care
• Provide clinical leadership while ensuring the delivery of high quality end-of-life care
• Review workflow, and monitor productivity
• Review and resolve documentation issues
• Ensure compliance with federal and state regulatory requirements
• Facilitate communication between families, patients, and healthcare providers

Requirements:

• Valid RN license in the state of Texas
• Hospice leadership experience (at least 1 year)
• Excellent documentation and clinical assessment skills
• Knowledge of home health/hospice regulations and general healthcare compliance
• Experience with Home Care Home Base (HCHB) EMR (preferred but not required)
• Excellent communication and people skills are essential
• Ability to work independently and as part of a team
